GracePoint Primary HealthCare, PLLC is a Direct Primary Care (DPC) practice. We are not health insurance and not a substitute for health insurance. A membership agreement does not qualify as minimum essential coverage under the Affordable Care Act. Patients are strongly encouraged to maintain health insurance coverage to pay for services not covered by this DPC membership.
The provision of Direct Primary Care Medical Services provided by GracePoint Primary HealthCare is not subject to regulation by the Texas Department of Insurance; is NOT a contract for health or accident insurance; and does not provide healthcare or accident coverage. It will only cover the services as described in the DPC Program Agreement. Patient should obtain or keep such health insurance policy(s) or plans that will cover Patient for general healthcare costs.
